NHS praise work of digital care coordinators

NHS HAMPSHIRE & IOW have praised the work of Digital Care Coordinators who are helping residents who have been digitally excluded to use the NHS App across the region; this recognition coming as part of a campaign to highlight different departments within the local healthcare services.

Digital Care Coordinators are members of staff who help patients get set up online and on the NHS app.  They can help patients install and use digital tools such as the NHS app and help them navigate the many features that are available.

More than 62 per cent of residents in Hampshire and Isle of Wight are using the NHS App to order prescriptions, make appointments, and view their test results. It is a great resource that gives residents a simple and secure way to access a range of NHS services at their fingertips; including the ability to book more appointments and mean they can be seen quicker.

It is further part of a national drive to ensure that more people have access to care within their communities and is written into the NHS ten-year health plan. Digital Care Coordinators are playing a huge part in the objective to move from analogue to digital by working within our primary care teams to help those who are digitally excluded.

Kay is based as a GP Practice in Hampshire in the Digital Care Coordinator role and said: “I have been working as a Digital Care Coordinator for the last year, and I really enjoy working with our patients to ensure that they get the best out of the NHS app and are able to use all of its features.”

“There are so many people in our community who struggle with technology and my role allows me to show them how to use the app to order repeat prescriptions, book appointments, view their health records and many other things. I see patients both at the practice and in their own homes helping them to realise the benefits of being able to log in and use the app to help with their medical care.”

The NHS app allows patients to view their GP health record and helps people to better manage their condition without having to phone their GP surgery. People can also access consultation notes, test results and future appointments.

Patients are being encouraged to switch on their notifications within the app, to ensure that they receive messages directly from their GP practice. Instructions on how to turn on notifications can be found here via this link.

Readers can start using the NHS App by downloading it on your smartphone or tablet via the Google play or App store. They can also access the same services in a web browser by logging in through the NHS website. However users must be aged over thirteen years old and be registered with a GP surgery in England or the Isle of Man, with more details here.

James Roach, Director of Primary Care and Local Care at NHS Hampshire & IoW said: “Our Digital Care Coordinators are becoming an integral part of our GP practices supporting those in our communities who have struggled to get online and use the services available via the NHS app.”

“Our GPs are seeing record numbers of patients each week and by having specialists like Kay in our practices, it ensures that patients can use the NHS app to keep themselves up to date regarding their medical treatment at their own convenience in their own home.”

The trust have been focussing on their Digital Care Coordinator this month as part of their campaign entitled ‘It Takes a Team’. It was launched in March 2025 and throws a spotlight on a different specialist role each month featuring people working within those roles.
